Rights & Permissions

Any individual, institution, or publisher who wishes to copy or excerpt material from one of Kolb Net Work's published books must request and receive permission from Kolb before the information can be used. All requests must be in writing.
The following information is required in order to process requests:

1. Title of book from which the information comes.
2. Page numbers in the book upon which the material requested appears.
3. Format the requested material will be reprinted in, or how will it be used.
4. Estimated number of pages the completed work will have.
5. Retail price.
6. Estimated date of publication
7. Name and mailing address of publisher.
8. Name and mailing address of person requesting permission
